Willowmere Advisory is an Arizona-based healthcare consulting and care coordination organization focused on strengthening transitions across the continuum of care.

We believe a successful healthcare transition requires more than a referral. Individuals navigating changes between providers, healthcare systems, and stages of care may encounter barriers involving access, transportation, insurance, social determinants of health, caregiver support, and the complexity of navigating an unfamiliar healthcare environment.

Willowmere was created to help healthcare organizations close those gaps.

Our approach combines care coordination, population health, healthcare operations, social work, business analytics, and performance improvement to develop transition solutions that are both person-centered and measurable.

Why Willowmere Exists

Willowmere was founded on a simple observation: healthcare organizations can do everything right in preparing someone for the next stage of care, yet the transition itself can still be difficult.

A referral may be placed, but the individual may still need help identifying an appropriate provider, understanding the next steps, arranging transportation, navigating insurance, addressing social barriers, or successfully establishing care within a new healthcare system.

Willowmere exists to help bridge that space between referral and connection.

Through collaborative partnerships with healthcare organizations, Willowmere develops care coordination and transition-navigation solutions designed to support continuity, improve access, identify barriers, and create measurable insight into what happens after the handoff.

Meet the Founder

Jessica Avalos, LMSW, MSBA, LSSGB
Founder & Principal Consultant

Jessica Avalos brings more than a decade of experience across healthcare, population health, care coordination, behavioral health, long-term care, and government programs.

Her experience includes supporting high-risk and high-utilizing pediatric populations, coordinating care across complex medical and behavioral health needs, addressing Social Determinants of Health barriers, navigating post-acute and community-based services, supporting interdisciplinary healthcare teams, and contributing to quality improvement, performance measurement, and program implementation.

Jessica holds a Master of Social Work and a Master of Science in Business Analytics and is a Licensed Master Social Worker and Lean Six Sigma Green Belt.

Her combination of social work and business analytics provides the foundation for Willowmere’s approach: understanding the individual experience of navigating healthcare while also examining the workflows, data, and systems that determine whether care coordination succeeds.
