Willowmere Advisory is an Arizona-based healthcare transition navigation and care coordination company dedicated to helping individuals and families navigate complex changes in healthcare with greater clarity, confidence, and connection.

Healthcare transitions can be challenging. Whether someone is moving from pediatric to adult healthcare, returning to the community after hospitalization, transitioning to a different level of care, or navigating unfamiliar healthcare and community resources, important connections can be lost along the way.

Willowmere was founded to help bridge these gaps.

Meet the Founder

Jessica Avalos, LMSW, MSBA, LSSGB
Founder & Managing Director

Jessica Avalos is a healthcare professional with more than a decade of experience in social work, care coordination, population health, behavioral health, long-term care, and government healthcare programs.

Throughout her career, Jessica has worked with individuals and families navigating complex healthcare needs, including children with chronic medical and behavioral health conditions, older adults transitioning between levels of care, Veterans, and individuals requiring specialized healthcare and community-based services.

Her professional experience includes coordinating interdisciplinary care, addressing social determinants of health, navigating healthcare systems, supporting individuals with complex needs, developing care coordination initiatives, and implementing quality improvement strategies.

Jessica holds a Master of Social Work and a Master of Science in Business Analytics. She is a Licensed Master Social Worker and Lean Six Sigma Green Belt.

Her unique combination of clinical social work experience, healthcare operations knowledge, and business analytics expertise provides the foundation for Willowmere’s approach.
